A surgical procedure is not the only way to safely end a pregnancy—medication abortion is also a safe, effective option for many people who are under 12 weeks pregnant. These medications can be prescribed by a licensed health care provider in person or online through a telehealth appointment.

The most common medication abortions in the U.S. involve taking two medications—Mifeprex (mifepristone) to stop the pregnancy from progressing and Cytotec (misoprostol) to cause contractions—on a specific schedule. This FDA-approved protocol is 92–98% effective in inducing an abortion and can be taken in a doctor’s office or at home (depending on the laws of your state).

In June 2022, the United States Supreme Court ruled that there is no constitutional right to abortion overturning Roe v. Wade and allowing states to make their own laws regarding abortion.

Bans and restrictions—particularly around surgical abortion—have since followed in dozens of states. However, it is still possible to obtain medication abortion pills in many places thanks to online abortion care organizations; demand for their services are expected to increase in the future.

It can be confusing to decide which service is best for you, so Verywell Health has created a list of reputable online providers to consider if you're considering a medication abortion.

While this is already lower than average, a pay-what-you-can option is also available for those with limited (or no) incomes.

Forward Midwifery has excellent client reviews. Still, it's worth noting that the service is small, with only one abortion provider—advanced practice midwife Christie Pitney MS, RN, WHNP-BC, CNM—and a clinical assistant. Pitney also fills prescriptions for Aid Access (listed above) in order to increase the number of people she can serve. Former patients also describe her as caring, non-judgmental, and kind.

Choix's website is easy to navigate and features resources like a blog, patient testimonials, information about emotional and practical/financial support, and fairly detailed FAQ page. There's also a comprehensive video about the abortion process to help you prepare, including what the medication looks like, how it's taken, and how to recognize abnormal side effects that require emergency care.

However, if you are located in a state with laws that protect abortion rights, carafem has an ideal mix of responsiveness, affordability, and convenience. With readily available providers, a mid-range cost, and the ability to schedule your appointment and receive your abortion pill medication quickly and discreetly, carafem is our top pick for the most comprehensive online pharmacy shipping abortion pills.

Guide to Finding Abortion Pills Online

Are Abortion Pills Still Legal?

Currently, two states ban the use of medication abortion starting at different points in early pregnancy: Texas at seven weeks, and Indiana at 10 weeks. However, many other states have placed restrictions on providers that make prescribing medication abortions difficult. Thirty-two states require providers who administer the medication to be doctors rather than nurse practitioners, midwives, or physician assistants. Nineteen states require the prescribing doctor to be present when the medication is taken.

Despite these restrictions, several organizations and pharmacies outside the United States will ship the pills necessary for a medication abortion to all 50 states. While this is good news for those in restricted states, it can take up to three weeks to receive the medication.

Comparing Online Abortion Pill Providers

If you're researching online abortion providers, you’ll want to consider several factors:

  • Price: The cost of a medication abortion varies depending on the provider you chose, starting as low as $110 or costing upwards of $700. Some providers accept health insurance, which can offset this cost, while other services offer financial aid and/or accept HSA/FSA cards.
  • Availability/states served: Most U.S.-based abortion providers are restricted to serving only a few states. If you are located in a state where abortion access is restricted or limited, you may need to choose a provider that is based outside the United States or travel to receive your abortion pill.
  • Doctors on staff: U.S.-based online abortion providers that prescribe medication abortions all have at least one doctor or other prescriber on staff. Foreign pharmacies that take direct orders from clients may not have a doctor available for consultation as some countries do not require a prescription in order to purchase an abortion pill.
  • Privacy protection: While most abortion providers take privacy seriously, think about what elements of privacy are most important to you, then check the company's terms and conditions. Consider things like how the medications are mailed and whether the packaging is discreet if you're concerned someone might go through your mail. If you plan to pay by credit card, consider how the charge appears on your statement. Some providers accept a variety of payment forms, including Bitcoin. For the majority of providers, you’ll need internet access, so take precautions with your search history and know how the company will use (or share) your personal data with third parties. All the online abortion medication prescribers on this list are privacy-conscious and use encrypted chat or video services.
  • Speed of delivery: Note how far along the pregnancy is and how long you’re able to wait for your medication to arrive. Some online abortion pill medication providers offer overnight shipping, while others might have shipping times as long as three weeks.

FAQs

Is medication abortion effective?

Medication abortion is considered highly effective. The FDA-approved, two-medication protocol has a 95% success rate at ending a pregnancy if taken before the 10th week of gestation.

Is medication abortion safe?

Yes, it is considered safe for most people. The rate of serious complications—including hospitalization for infection or transfusion—is less than 0.4% if taken by the 10th week of pregnancy. The risk of death associated with childbirth is approximately 14 times higher than that with abortion.

When is it not safe to take medication abortion pills?

It is not safe to take these medications if you are unsure if you are pregnant, if your pregnancy is further along than 12-14 weeks, if you have an ectopic pregnancy, or if you have certain health complications that make heavy bleeding more likely. Discuss any concerns you have with your health care provider during your telehealth visit or chat.

What are some of the side effects of medication abortion?

Bleeding, cramping, nausea, diarrhea, and stomach upset are all common side effects. Some online abortion pill providers will include medication to help with the discomfort associated with taking abortion pill medication. While rare, it is possible for some people to experience excessive bleeding after taking the abortion pill. This is considered a medical emergency and you should seek urgent medical care.

How will I know if I need to go to the hospital after taking an abortion pill?

If you are bleeding so much that you soak two or more maxi pads in an hour for two hours in a row or if you're passing blood clots larger than a lemon, seek medical care right away. You do not need to disclose that you took abortion medication if you do not want to, as incomplete medication abortion and natural miscarriages have similar symptoms. The medication can't be detected in your bloodstream when taken orally.

Methodology

This list was compiled after evaluating dozens of online abortion pill providers. In order for them to make our list, they first had to be reputable and safe. We confirmed this by making sure that all our providers were verified by either the National Abortion Federation, Planned Parenthood, or Plan C, a nonprofit that has ordered abortion pills from all providers listed on their site to ensure that they were not scams.

We also made sure any prescriber we recommended had policies in place to protect patient privacy. We looked for services that offered discreet shipping, encrypted patient-prescriber communications, quick browser exit features, minimal site cookies, or more.

As a U.S.-based publication, we favored abortion care organizations with pharmacies located within the country due to quicker shipping times. (While international pharmacies are not inherently untrustworthy—some do sell authentic medications—their products may be manufactured to different standards than required by the FDA.)

Finally, to make our list, online pharmacies or prescribers must ship to multiple states, offer reasonable prices (or financial aid), and provide support resources. We also prioritized those that offered quick shipping options or had doctor- or nurse-operated phone lines for questions or concerns.
